I have enrolled in a program of strenuous physical activity including, but not limited to deep water exercise provided by Fluid Running LLC. I hereby affirm that I am in good physical condition and do not suffer from any disability that would prevent or limit my participation in this exercise program, including travel to, from and during this activity.
In consideration of my participation in the Fluid Running® exercise program, I, for myself, my heirs, and assigns, hereby release, indemnify and hold harmless Fluid Running LLC, their affiliates, officers, managers, members, agents, attorneys, staff, volunteers, heirs, representatives, predecessors, successors, assigns, its instructors, and any other facility/pool where Fluid Running classes are being offered, from any claims, demands, and causes of action arising from my participation in the exercise program.
I fully understand that I may injure myself as a result of my participation in the Fluid Running® exercise program and I hereby release, indemnify and hold harmless Fluid Running LLC, their affiliates, officers, managers, members, agents, attorneys, staff, volunteers, heirs, representatives, predecessors, successors, assigns and its instructors from any liability now or in the future including, but not limited to; heart attacks, stroke, muscle strains, pulls or tears, broken bones, heat prostration, knee/lower back/foot/hip injuries, and any other illness, soreness, or injury however caused, occurring during or after my participation in the exercise program.
I irrevocably grant Fluid Running LLC permission to use and own all rights, including the copyright, to any photograph, videotape, digital image, or other likeness of me taken while participating in any Fluid Running program.
I hereby affirm that I have carefully read this waiver and release and fully understand that it is a release of liability.
